Self-Assessment is critical in determining how much one has achieved and how much more remains. As a nurse practitioner, much is expected in practice. One is looked upon by the RNs and the students. To be a good role model, one has to develop good skills. Progress or stagnation can only be noticed when one performs a self-assessment test. An assessment test also allows one to denote established strengths and realize weaknesses, and it also enables one to devise ways to boost the strengths and fix the weaknesses. This paper aims to determine the progress, strengths developed, areas of weakness, and how they can be mitigated.

The Clinical Skills and Procedures Self-Assessment test score from week one revealed weaknesses in physical examination skills, health history taking, and developing patient care plans. I was still green in the field, and I had developed minimal nursing skills. My communication skills were still poor and undeveloped as of week one. The four areas are very crucial to any nurse during practice. I developed goals to help me grow in my practice regarding these areas.  I focused on mastering health history, performing physical examinations, improving communication skills, and developing patient care plans.

After the evaluation test, I realized I had added expertise in developing a patient care plan, communication skills, and performing a physical examination. I realized I am well-versed in nursing theory, and thus developing a nursing care plan for any patient assigned was relatively easy. The care plans developed produced better nursing care to the patients translating to better health outcomes. A Patient care plan is a crucial tool in the management of any patient in the hospital.

Elaborate and exclusive care plans are the cornerstone of better patient health outcomes (Richards et al., 2018). A physical examination is a vital tool in developing a diagnosis. Physical examination skills are essential requirements, and these skills have been instrumental in accurately diagnosing patients and creating a nursing care plan (Hypman, 2020). Routinely practicing performing physical examinations and developing patient care plans improved my skills over time.

Communication is essential in all activities in the hospital. To maintain a healthy relationship with the patients and other healthcare providers, one requires good communication lists. Studies have associated poor communication skills with medication errors that often lead to patients’ deaths, and thus, communication is vital (Arnold and Boggs, 2019). I revised my communication skills and familiarized myself with hospital information systems. These helped me achieve good communication skills. Taking health history posed a significant challenge to me as I saw it to be time-consuming. Most times, there were very few patients eligible for admission into the hospital.

Learning never ends, and the areas I gained expertise in and the areas I am still struggling with all require much input. In my future practice, I intend to take patients’ health histories, perform a physical examination, and develop a care plan for the patient. Completing the three on a single patient shall ensure consistency in my work, patient follow-up is more manageable, and measuring my progress is also possible (Richards et al., 2018). Tracing patients and doing follow-up is difficult when one uses different patients, as I realized in my previous practice. This shall ensure I work on the goals simultaneously, achieving harmonious professional development. I shall also revisit the care plan and physical examination guidelines to ensure I conform to the standard practice. More so, I shall teach my colleagues and the students physical examination skills because teaching them shall also enhance my skills (Hypman, 2020).

Communication involves many media such as written work, and thus, I shall be doing clear and legible documentation of health history, physical examination, and patient nursing care plan. Communicating findings and other important patient health information to other healthcare providers and the patient shall ensure effective care collaboration and improve patient outcomes (Arnold and Boggs, 2019). When patients are informed and actively involved in their care, they most likely cooperate. Healthcare is very dynamic. I shall ensure I am informed when guidelines or other changes in healthcare practices are made to ensure my practice is up to date. I intend to practice active listening to both colleagues and patients because active listening determines the context’s understanding and informs one’s decisions (Arnold and Boggs, 2019, Reassessment of Strengths and Opportunities Essay).

To improve my health history-taking skills, I shall revise the guidelines on health history taking and ask my colleagues to assist me in history taking. I plan to use my free time to go through patient cases and made patient histories. The step shall ensure I develop the skill. Health history and physical examination provide cues that are the basis of developing patient care plans. With a good, clear, and elaborate health history and a well-performed physical exam, the diagnosis is most likely accurate. Thus, the care plan developed produces better patient health outcomes.

Looking back is very critical in determining the next moves. Without looking back, it may hard to move forward effectively. Thus, any practicing nurse must evaluate their progress, define their strengths and weaknesses, and determine their way forward. That way, they can work on their weaknesses and build on their strengths on their journey to professional development.
