Introduction:  The following is a recording of a conversation between a client that is a 60 year old female, and was recently widowed about six months ago. The conversation began by me commenting on the state of her pool, then the client began talking about the condition of the backyard and how it used to look before hurricane Irma, and how it needs to be renovated now. The conversation continues to unfold and the client begins to communicate about her husband that passed away and her feelings about it.

I said: “Oh no, the pool is starting to get green.” My initial comment: “Oh no, the pool is starting to get green.”- Is a non-therapeutic form of communication as it can be perceived as an unnecessary form of disapproval. I could have said to the client, “I can’t wait for it to be warm enough to go into the pool, it was so nice last September,”- This would be an encouraging comparison for the client to reference and help the client to uncover new resolutions.
The client: “I really need to get the pool together, I hope the new filtering system comes soon”.

 

The client seemed concerned about the pool, her face did not show any expression of happiness or sadness, just a “flat” expression.
I replied: “It should be here soon and we can make it a weekend project.” My reply: “It should be here soon and we can make it a weekend project.”- Is a non-therapeutic form of communication because it is a false reassurance. I could have asked the client, “It sounds like you are worried about the time it will take for the filter to be delivered”- this would have presented a broad opening for communication about the pool.
The client: “I hope so, the yard hasn’t been the same since Irma. I really need to get the pool better and the back porch fixed- The roof needs to be put back on and some of the boards need to be replaced.” The client seemed worried about the condition of the yard, her facial expression and vocal tone seemed stressed. The client was observing the yard and not making much eye contact while talking to me. 
I replied: “That sounds like a lot of work.” 

 

 

 

My reply: “That sounds like a lot of work.”- Is therapeutic communication because it is a broad Segway for the client to communicate more about the work that needed to be done.
The Client: “Yes, but we are going to do it. I will come along.” The client’s verbal response was positive, acknowledging that there was a lot of work to be done- but it would all come together. Her eye brown soften and her facial expression seemed less tense.
I replied: “One step at a time, we will get it all done.” My reply: “One step at a time, we will get it all done.”- Is therapeutic communication, as I am offering “self”, giving the client my time, and showing that I value her feelings of wanting to fix the yard. Process Recording Paper
The Client: “This backyard used to be where we would hang out. I think he (her husband) must have been feeling bad for a really long time. I never knew he was sick.” The Client seemed distraught, her eyebrows were peaked midline and her eyes looked sad. She crossed her arms- that body language gave off a self-comfort and slightly closed off vibe.
I replied: “There is no way you could have known he was sick, he was so strong.” My reply: “There is no way you could have known he was sick, he was so strong.”- Is therapeutic communication because it acknowledges/accepts the client’s feelings about not knowing her husband was sick, and gives recognition to her husband for being so strong.
The Client: “I know; but, I feel like I should have noticed something was wrong. The client uncrossed her arms and legs and propped her shoulders up with both hands on her knees and looked back out to the yard again. Then looked back at me as if she was waiting for me to respond. Giving me a kind-of signal that she wanted me to say something. Process Recording Paper
I replied: “It is hard to notice things like that, I mean, what could you have known then” My Reply: “It is hard to notice things like that….”- Is therapeutic communication because I was focusing on the client’s previous situation, and giving the client a broad opening to communicate about what she thinks she could have done.
The Client: Stopped talking… and sat silently.My response: I sat silently with her. The client looked as if she was deeply thinking about what I had asked. She didn’t say anything, she never answered what I asked. So, I just sat with her, incase she decided she wanted to talk about it and to offer myself to be present as a sign of solidarity.- This non-verbal communication is considered therapeutic.

Summary:

To summarize, I used both therapeutic an non-therapeutic communication during the conversation with the client. The process recording activity, collecting of a dialog of a general conversation, and examining my style of communication facilitated me in identifying how often my communication style is non-therapeutic.

Therapeutic communication is important, it is important to a client in your care, your friends, and your loved ones- it instills a sense of trust, reliability, and accountability. As a nurse, I will need to put into practice the use of effective therapeutic communication in my daily personal life and my professional life.

Understanding non-verbal communication and being receptive to body language and facial expression cues from a person is a key component in knowing how to apply therapeutic communication, it is important to be attentive when communicating. Utilizing therapeutic communication in day-to-day life situations and professional/healthcare settings will improve my overall communication skills, allowing me to be a well-rounded and open communicator in any situation.
