According to Cather et al. (2017) 64 to 79% of schizophrenic patients are concomitant tobacco smokers. This pose a risk to their general health since tobacco smoking is associated with high rates of cardiovascular disorders and cancer among other risks. The Psychiatric Mental Health Nurse Practitioners (PMHNP) who looks after these clients invariably want to avoid these risks as well as rehabilitate these patients. To achieve these goals however, the PMHNP are confronted with a number of challenges which are going to be explored in the subsequent paragraphs of this paper. One of the greatest challenges facing the PMHNP when thinking of assisting the affected patients is lack of effective communication. Proper communication between the patient and the nurse is a prerequisite for good patient nurse relationships as well as therapies such as personal and cognitive behavioral therapy (Mangano et al., 2020). Additionally, work place violence associated with smoking schizophrenic patients due to aggression pose a threat to the confidence levels of PMHNP when it comes to offering the necessary care (Khazaie, Ahmadi & Maroufi, 2017 Week 5 Discussion: Schizophrenia and Tobacco Use).

Despite having enormous skills and knowledge of psychiatric care, many desist to offer this individual care to protect their safety. In spite of smoking, nicotine dependence and schizophrenia being conditions that can benefit from psychopharmacotherapy, this benefit is not often achieved as desired because of the low rates of treatment adherence (Curtis et al., 2018 Week 5 Discussion Schizophrenia and Tobacco Use), not to mention the difficulty in monitoring that nurses experience with these clients. 

PMHNP also face obstacles of handling these victims due to the nature of schizophrenia. Nevertheless, to quit smoking or nicotine dependence for example, one is required to make a decision to stop the habit then deal with the triggers. PMHNP therefore cannot effectively advise and initiate the rehabilitation process as these clients have lost insight, self-awareness and concurrently poor decision makers and socially withdrawn.
