Summarize the Patient, Family, or Population Problem.

Globally, hypertension is one of the chronic diseases that affect modern health. Hypertension is a medical disorder characterized by a blood pressure greater than 140 mmHg in the arteries. Because of the elevation, the heart has to pump blood faster than usual to circulate blood via the blood vessels. Many adults have hypertension and are unaware of the terrible ailments that might arise.

Intervention and Analysis

African Americans are the most affected population in the United States of America because of various predisposing factors such as lack of exercise and racial discrimination at health care organizations. Recent empirical research from the Centers for Disease Control and Prevention (CDC) shows that 41% of people of color have developed high blood pressure (Nakamura, 2021). There is a dramatic difference compared to the white as they are only 27%.

Health professional bodies such as World Health Organization (WHO) have come up with different health interventions to help reduce and eradicate hypertension. It is now one of the chronic diseases that causes massive losses.

The creation of a teaching plan for African Americans is one of the most appropriate health interventions that can be deployed to help in reducing and eradicating hypertension amongst people of color. The gist of this paper is to explore the use of a teaching plan as a health intervention to fight against hypertension.

Define the role of leadership and change management in addressing the problem.

I chose hypertension as a health care problem because the issue has not been given enough attention. The government and other health care professional bodies have not worked on the issue, and it is affecting the majority of the population, especially African Americans. Also, very little research has been done on this problem, and I feel that I can have a broad scope of information while coming up with the intervention.

Furthermore, African Americans experience health care disparities, which is why enough attention has not been given to this issue. In this context, I chose high blood pressure as a health care problem because I would like to save people of color from developing this problem.

A teaching plan strategy is directly connected to the African Americans as they are the most affected and experience health care disparities. The intervention will be of great significance to Black Americans as it will reduce and control hypertension through preventive measures such as exercise.

Explain why the problem is relevant to your professional practice and the patient, family, or group.

The population being focused on are African Americans, and this problem is relevant to them because they are the most affected. CDC data shows that people of color are the leading population suffering from high blood pressure (Nakamura, 2021).

Thus, African Americans would benefit from this health issue since they would understand the most appropriate intervention that they can deploy to reduce and control hypertension. Also, the population will understand the type of lifestyle to lead to avoid developing such health complications.

On the other hand, high blood pressure works best for my professional practice since it would help me develop skills and knowledge on reducing and controlling hypertension.

Explain how leadership and change management strategies influenced the development of your proposed intervention.

Leadership plays a vital role in the health care setting since it is involved in decision-making. Using a teaching plan as an intervention for reducing and controlling high blood pressure can only be successful if the leadership team makes effective decisions.

The leadership team has the role of ensuring that they make effective decisions concerning the health intervention and give direction on how to approach the issue and the expected outcomes (Chreim & MacNaughton, 2019). In this context, the leadership group expects the subordinates to follow all the decisions made and achieve the set goals.

In this case, another role of the leadership team would be to assign different tasks to different stakeholders as far as the intervention is concerned. For instance, one health care provider can be selected to address the following using a teaching plan.

If one is overweight or obese, they can consider losing some pounds to control their blood pressure. Being overweight leads to difficulty in breathing which increases blood pressure. Watching your weight will help reduce hypertension. People are also affected by their lifestyle and the types of foods they eat (Nakamura, 2021). Eating a diet rich in fruits, whole grains, low-fat products, vegetables, and low-fat dairy products can help lower your blood pressure.

Having a reduced amount of sodium in the meals also helps lower blood pressure. Not adding salt into your food can be controlled after preparing and avoiding processed foods. Most natural food does not contain sodium, and it is always added during processing. Reading labels before purchasing processed foods can help you choose the one with lower sodium. Increasing the intake of potassium, on the other hand, reduces the effects of sodium on blood pressure. Fruits and vegetables are the primary sources of potassium.

Change management is significant in any organization since it enhances the easy achievement of individual and organizational goals. Creating a teaching plan would require the healthcare organization to embrace change management because the teaching plan has to be integrated into the healthcare organization’s activities.

The management team would change its activities to ensure that the intervention plan is also embraced. One of the most critical areas that would require change management is the technological sector. In most cases, hypertension is managed using mobile phones, which African Americans would be enlightened of how to use them.

This would require the management team to make several changes in the technological sector to ensure that all people of color can access the teaching plan. For instance, the teaching plan can address mobile apps that can be used to manage and control high blood pressure. These can be achieved through change management strategies that the management team can implement.

Explain how nursing ethics informed the development of your proposed intervention.

Nursing ethics are considered the development of my proposed intervention since they advocate for the benefits of the patients. One of the nursing ethics that informed the creation of a teaching plan was beneficence. This nursing ethic advocates for kindness and charity from the health care provider to benefit the patient (Nakamura, 2021).

I developed the intervention plan by ensuring that the nurse, in one way or another, would benefit the patients by showing kindness and charity. This nursing ethic is significant because many health care interventions should ensure that nurses act as advocates of patients during rendering health care services.

Autonomy is another nursing ethic principle that informed the development of my intervention because patients have the right to make decisions that they think are good for them and promote their well-being. For instance, the main critical factors considered are to ensure the teaching would be successful are the cultural and religious beliefs.

This is because different communities have various beliefs, and understanding them would help us understand and develop trust between us. Each person should be treated as an individual rather than being grouped by their culture. As most people are not aware of managing hypertension, providing them with a teaching session would benefit them.

Propose strategies for communicating and collaborating with the patient, family, or group to improve outcomes associated with the problem.

It is important to note that communications play a significant role in any organizational setting since it determines internal and external collaboration. According to the qualitative research from CDC, strategies for communication and cooperation enhance the easy achievement of any health intervention goals.

Other nursing literature reviews suggest that effective strategies for communication and collaboration are instrumental in any health intervention as they enhance effective interaction between the health care providers and patients.

Verbal communication is one of the communication strategies used because nurses interact with African Americans face to face. The nurses would be expected to communicate to the patients about the intervention using verbal communication when the patients visit health care organizations. Simple language should ensure that all patients understand what the intervention is all about (Graves et al., 2018).

For instance, the nurse can use verbal communication to address chronic stress as one of the primary causes of high blood pressure. “Chronic stress can also lead to hypertension. Stress can be caused by family issues, friends, work, illness, or even finances. One should determine their causes of stress and develop mechanism on how they can deal with it.”

Written communication would also be deployed. Written communication would be done through mobile apps, where patients can access the created teaching plan on their phones. This would be the most appropriate communication because of the technological advancement implemented in the health care system.

Patients will be able to access information without any physical movement, which will be cost-effective. Patients and health care providers would find it easy to collaborate because of the effective communication strategies in place.

Discuss the benefits of gathering their input to improve care associated with the problem.

Gathering information from African Americans is important because I would get first-hand information on the problem. Patients will share accurate information about hypertension because they are the victims. Gathering information from other populations is likely to be inaccurate because it does not impact them.

In brief, getting information from people of color will lead to a successful intervention as accurate information will be collected.

Identify best-practice strategies from the literature for effective communication and collaboration to improve outcomes.

Technological communication would improve the outcomes since all African Americans own mobile phones. Technological communication is the most effective because it reaches all patients as fast as possible. Also, using the mobile app is efficient since one sender can send it to as many patients as possible.

Effective collaboration and communication will be achieved because all health care systems have integrated technology in their systems (Wang et al., 2018). Patients have also embraced technology using mobile phone apps to communicate with healthcare providers.

Explain how state board nursing practice standards and/or organizational or governmental policies guided the development of your proposed intervention.

The National Commission on Correctional Health Care’s Standards guided the development of the intervention by ensuring that health standards of African Americans were maintained. The body advocates for access to health care services, improved nursing care, and staff and patient safety (Oldman, 2021).

This professional body was of great significance since it advocated for improving African American health. The professional body ensured that all African Americans could access the teaching plan to reduce and control hypertension. It was easy to develop the intervention since the National Commission on Correctional Health Care’s Standards fully supported it.

Explanation of how technology, care coordination, and the utilization of community resources can be applied in addressing a patient, family, or population health problem.

Technology is one of the most crucial resources since it will be used for communication. Smartphones will be used where all communication will be made. The patients will be expected to access apps on their smartphones to get all the information regarding their health.

Care coordination will be of great significance since health care providers will be expected to share various duties to ensure that the intervention is successful. Care coordination will enhance the easy achievement of goals as different expertise will be shared (Nakamura, 2021). Lastly, different community resources such as classrooms will be utilized to teach African Americans how to reduce and control hypertension.

The population will be expected to use all available community resources to ensure that the intervention enhances the quality of life and well-being. In brief, technology, care coordination, and community resources will be incorporated into this intervention to ensure that high blood pressure amongst African Americans is reduced and controlled.
