Complex situations in the healthcare industry requires highly educated, skilled and trained nurses to be death with effectively. Advanced practice registered nurses (APRNs) are tasked with the duty of providing advanced patient care as well as supervising other staff. Advanced practice enables physicians to pay greater attention to patients of higher complexity and acuity (Molassiotis et al., 2021).    Moreover, it helps nurses to take on greater roles and specialties essential to the advancement of public health. It is a requirement that APRNs obtain at least a master’s degree and further specializations within the sphere of specialization.

Advanced Nursing and Nursing Philosophy

Every nurse identifies with only specific models that work best for them considering their objectives and nursing philosophies. I choose Watson’s theory because it offers the best results for patients, that is care, love, empathy and quality services. According to Jean Watson, caring regenerates life energies and potentiates our capabilities. In addition, it emphasizes quality care, empowerment, creating awareness, cooperation with patients and meeting the needs of the patients. The theory has four major concepts; human being, health, environment/society and nursing.

Prescriptive Authority (Texas)

Prescription of medication in Texas is done under strict guidelines as provided for in section SB 406. Section SB 406 allows physicians to delegate the duty of prescription of prescribing medication to nurse practitioners based in any location in Texas. Furthermore, physicians have the power to delegate the work of ordering and prescribing prescription drugs, nonprescription drugs, medical devices and medical equipment to a physician assistant and APRNs. Also, all APRNs can prescribe all drugs in schedule II-IV with very few limitations.

Prescriptive Authority (California)

Prescriptive laws in California are highly regulated and very strict. In California, all registered NPs to Nurse practitioners have the authority to provide basic primary care. However, NPs in California cannot prescribe medicine without authorization from a supervising physician. This policy requires nurses to have an agreement with a doctor before being allowed to prescribe medicines. California NPs can prescribe all drugs and medications in schedule II-V inclusive of all the highly addictive drugs in schedule II.

Pros and Cons for Each State

In both California and Texas, all NPs have the right to prescribe even the most sensitive drugs such as schedule II drugs. Also, in both states, the NPs have the capacity to order for drugs even remotely without having to be physically present though they are required to have an active practicing license prescriptive registration number. NPs in the state of California and Texas can prescribe just about any drugs as long as it is authorized by the supervising doctor. However, both states do not allow their NPs to prescribe medicines/drugs without authority from a physician.

Discussions on Changes and Future Expectations

In California, there has been a spirited fight by nurses to have limitation on prescription lifted. The NPs in California work alongside doctors through standardized protocols that require collaboration, signatures and approvals. In my view, these protocols are critical in ensuring patient safety but I also feel that there are some prescriptions that nurses can make without needing the approval of physicians. For example, APRNs who are highly trained educated and experienced nurses can accurately prescribe some medication on their own based on the years of education and experience they have in the field (O’Connor et al., 2018). Having to wait for approval from the doctors is sometimes painful for nurses because sometimes they see situations that they can handle effectively but have to wait for approval which sometimes takes long depending on the doctor’s schedule.

Advanced Nursing and The Health Organization

Health organization requires attitudes that enhance employee performance in addition to a culture of linguistic competence. Linguistic competencies help organizations to relay information in a way that it is easily understandable to a diverse audience. Not to forget, effective communication creates more cohesive teamwork leading to enhanced productivity. From advanced practice, I learned that health organizations need to identify how culturally diverse the communities they serve are since the cultural beliefs, vulnerabilities, strengths and values of different cultures influence their operations and profits (Kim et al., 2021). The organization should involve the advanced registered nurse practitioners when it comes to suggestion, formation and implementation of health policies and this involvement will require knowledge of legislation that affects the healthcare and APRN practice.
