The Selected Model and Rationale

My model of choice is the Advancing Research and clinical practice through close collaboration (ARCC) model. ARCC is a step-wise evidence-based practice (EBP) model for advancing the implementation for change and EBP sustainability through the use of EBP mentors (Melnyk et al., 2017 NUR590 Benchmark – Evidence-Based Practice Proposal Project: Framework or Model for Change). The EB mentors enhance organizational and clinicians` belief that is paramount in strengthening organizations` cultures and facilitating implementation and sustainment of an EBP.

This model has been used in various centers and organizations with a result indicating increased job satisfaction, reduction of burnout, improved patient outcome, and reduction in the cost of care upon implementation of an EBP (Huybrechts et al., 2021). Likewise, I choose this model with the intent to improve patient outcomes through the implementation of daily bathing of central line with chlorohexidine to reduce the rate of central-line associated bloodstream infections (CLABSI). Furthermore, I believe that using EBP mentors will improve clinicians’ beliefs, enhance organizational culture for EBP, support EBP implementation, and enhance patient experience and outcomes.

 Description of Stages of ARCC and Application

The ARCC model is built on five major steps that ensure stepwise implantation of an EBP and its sustainability to achieve quality outcomes (Melnyk et al., 2017). The first step is the assessment of organizational culture and readiness for the implementation of change.  In this phase, various factors are considered including the administrative policies, resources, time, and availability of personnel to implement change. In addition, the mission and the vision of an organization dictate the beliefs and the culture of an organization, both of which are paramount in defining the aspirations of an organization. This assessment is also useful in identifying organizational weaknesses and strengths. For this project, the TeamSTEPPS assessment tool will be used in assessing organizational readiness for change. In addition to assessing organizational weaknesses and strengths, the tool also assesses the capacity and organizational motivation to implementation of change. Based on the tool, Houston Methodist has supportive leadership guided by informed beliefs, mission, vision, teamwork, interprofessional collaboration, and mutual support which are imperative in the implementation of change. The second step is the identification of both barriers and facilitators of the EBP process within the organization. Identification of barriers is essential in coming up with constructive ways of overcoming them to enable the implementation of an EBP (Melnyk et al., 2021 NUR590 Benchmark – Evidence-Based Practice Proposal Project: Framework or Model for Change). Facilitators, on the other hand, form a framework of factors that will enhance EBP implementation. The known barriers to EBP implantation include lack of skills and knowledge about EBP, the misconception that EBP takes much time, lack of administrative support, insufficient EBP mentors, inadequate resources, and the traditional notion of educating nurses on how to research rather than apply the research into clinical practice. Facilitators include supportive administrators, a collaborative working environment, and EBP approval, and improved workplace relations. I have considered both factors to ensure the effective implementation of EBP.

The third step is the identification and establishment of EBP mentors who will work directly with staff at the point of care thus fostering beliefs, skills, and EBP knowledge (Melnyk et al., 2021). Furthermore, they strengthen clinicians’ beliefs about EBP hence encouraging the adoption and implementation of an EBP practice. In this project, I intend to identify EBP mentors from various sectors including nurse managers, clinical educators, nurse researchers, clinical nurse specialists, and senior clinical staff within the Houston Methodist. The identified mentors will be trained through workshops, simulations, and tutorials to improve their skills and knowledge on EBP and enhance its implementation. The fourth step is the implementation of the evidence into practice. The implementation ensures the use of the best available clinical evidence to guide informed decision-making regarding patients’ care (Yoo et al., 2019 NUR590 Benchmark – Evidence-Based Practice Proposal Project: Framework or Model for Change). Nurses will be involved in the implementation of the EBP practice under the guidance of EBP mentors. The implemented practice will be guided by research findings, the clinical expertise of the attending healthcare providers, patient preference as well as values and ethics. In this project, all nurses providing ICU will be trained on safe bathing of central lines and encouraged to routinely bathe central lines to reduce rates of CLABSI.

Finally, evaluation of the outcomes from the practice change is the fifth step. Evaluation is essential in determining project relevance while determining the level of achievement in terms of objectives, efficiency, effectiveness, impact, and project sustainability (Melnyk et al., 2017). Furthermore, evaluation ensures that limited resources are utilized efficiently to attain the greatest impact. Also, adjustments are made during evaluation to improve an EBP. In this project, the evaluation will be determined in terms of hospital stay, cost of care, patient satisfaction, healthcare provider satisfaction, cohesion, and patient turnover. The expected outcome is improved patient outcomes through the provision of quality care.
