1b.  Summary of your advocacy action plan to address your identified SDOH

  • Facilitate open dialogue with patients about symptoms and self-care habits.
  • Give patients and their family members a wide latitude for asking questions.
  • Act with kindness when delivering competent care to all walks of life.
  • Address the unique religious and/or cultural needs of patients.
  • Incorporate ethics and legal knowledge into nursing care. Clinical Practice Experience CPE Record
  • Integrate scientific evidence and patient/family preferences into nursing care.
  • Understand the influence institutional policies and government have on public health.

1c. One (1) goal in SMART format that provides unified direction for the Advocacy Action Team.

  • To increase cancer awareness among influential groups and the public;
  • to reduce stigma and fear of cancer in the low socioeconomic groups;
  • to engage and mobilize key stakeholders within the cancer community who will champion the development and implementation of a comprehensive national cancer control plan and its components;
  • to progressively expand advocacy groups, including community volunteers and patients groups, to cover all the regions or provinces of the country;
  • to promote the value of comprehensive cancer control and the need for developing policies and programs;
  • to launch a simplified version of the comprehensive cancer control plan and disseminate it widely to the media and the lay public using attractive messages; Clinical Practice Experience CPE Record
  • to promote the effective and equitable implementation of key priority (core) interventions;

    1d.  List of policymakers and contact information

  • Patients. Patients are typically citizens, and voters, and sometimes taxpayers. Policymakers have a fiduciary duty to this population, and the country’s policy framework is established to benefit patients. Patients receive care services from providers and are the beneficiary customers of the payors. Patients also may want to access information about their care via an electronic device (e.g., personal computer, mobile phone). Clinical Practice Experience CPE Record
  • Providers. Providers operationalize care delivery within the policy framework. They provide health services to patients and maintain health information about them. The providers coordinate patient care with other providers as care team members. Many providers are independent businesses that must manage their own operations and finances.
  • Payors. Payors operationalize the financial elements of the policy framework. Payors enroll patients as beneficiaries. They procure care services from the providers on behalf of their patient beneficiaries. They also must take on the actuarial task of ensuring the financial sustainability of the care program. They report to policymakers. Clinical Practice Experience CPE Record

    1f. Written Reflection Summary of your Phase 1 experience

As the nurse leader, there are a few techniques available to me to make up for the variance while still complying with set regulations. One method that I will use to balance the variance is to reduce the number of hours per shift for all FTEs during the night shift and day shift. Instead of the 12-hour shift, the FTEs will only do a maximum of 9-hour shifts for the month. Every shift will still have at least 8 RNs in the surgical department. The number of assistant nurses remains as it was before (3), for both night and day shifts. Alternatively, I can increase the use of part-time nurses while reducing the number of FTEs. The idea here is to curb the ballooning wage of FTEs by capping their numbers per shift and the number of hours they work per shift until the variance is balanced. 2a.𑂽 ●       List and describe seven team-building strategies from course material to effectively form and lead your interprofessional policy advocacy team. Clinical Practice Experience CPE Record Provide a Platform for Social InteractionGet your interdepartmental teams talking and collaborating through corporate citizenship activities.Reward and RecognizeInevitably most job performance goals are assessed on an individual level, however consider incorporating team-oriented rewards or goals.

Integrate Collaboration into Day-To-Day Tasks and Project

Integrating teamwork into everyday tasks will open dialogue and encourage knowledge sharing

Incorporate Team Building Activities

Another interprofessional collaboration strategy is to, where possible, incorporate team building activities. Clinical Practice Experience CPE Record

5.Forming

6. Norming

7.Storming

Explain why each of the seven strategies is important in effective team building.

1. BUILDS TRUST

Trust plays an essential role in building effective teams. For teams to work together, they need to know they can trust each other. That they can fall back on each other if the need arises. Moreover, when you build trust among teams, they give each other space and autonomy to accomplish their tasks and make their own decisions. Clinical Practice Experience CPE Record

Trust makes people feel safe. When they feel safe, they open up. They let their team members know about their strengths and weaknesses. They are more proactive with their ideas, take risks, listen to each other and then arrive at a consensus. As a result, there’s more collaboration, communication and team members aren’t afraid to expose their vulnerabilities to each other. Clinical Practice Experience CPE Record

2. REGULATES COMMUNICATION

When employees work as a team, they communicate. They talk to each other about the task at hand and the best way to achieve the desired result. They strategize, divide themselves into smaller groups, hold discussions and try to finish the task in the most efficient manner. Communication also allows employees to understand their roles and what their peers are doing. When employees know what their team members are doing, they can check up on the progress made and help each other out if someone cannot reach their goal.

3. INCREASES PRODUCTIVITY

Teams share the workload. This means that if one member has relatively less work, she can help another team member complete their work. This allows the project to be finished faster, thereby increasing productivity and improving the overall bottom line. Clinical Practice Experience CPE Record

The importance of team building is that it improves the individual’s and the organization’s productivity. Because individuals work in teams, they can pick up on new skills and sharpen their existing skills. This improves team performance, makes them efficient and over time, more work gets completed in less time. As a result, organizations can generate more revenue when they achieve their targets and deliver their best.

Health advocacy or health activism encompasses direct service to the individual or family as well as activities that promote health and access to health care in communities and the larger public. Advocates support and promote the rights of the patient in the health care arena, help build capacity to improve community health and enhance health policy initiatives focused on available, safe and quality care. Health advocates are best suited to address the challenge of patient-centered care in our complex healthcare system.

The Institute of Medicine (IOM) defines patient-centered care as: Health care that establishes a partnership among practitioners, patients, and their families (when appropriate) to ensure that decisions respect patients’ wants, needs, and preferences and that patients have the education and support they need to make decisions and participate in their own care. Patient-centered care is also one of the overreaching goals of health advocacy, in addition to safer medical systems, and greater patient involvement in healthcare delivery and design. Clinical Practice Experience (CPE) Record

Patient representatives, ombudsmen, educators, care managers, patient navigators and health advisers are health advocates who work in direct patient care environments, including hospitals, community health centers, long term care facilities, patient services programs of non-profit organizations or in private, independent practice. They collaborate with other health care providers to mediate conflict and facilitate positive change, and as educators and health information specialists, advocates work to empower others. Clinical Practice Experience CPE Record

In the policy arenas health advocates work for positive change in the health care system, improved access to quality care, protection and enhancement of patient’s rights from positions in government agencies, disease-specific voluntary associations, grassroots and national health policy organizations and the media. Clinical Practice Experience CPE Record

There may be a distinction between patient advocates, who work specifically with or on behalf of individual patients and families, or in disease-specific voluntary associations, and health advocates, whose work is more focused on communities, policies or the system as a whole. Often, however, the terms “patient advocate” and “health advocate” are used interchangeably. Clinical Practice Experience CPE Record

2b.𑂽 ●       Consider the expertise you will need on your Advocacy Action Team to achieve your SMART goal then:Patients-Patients have an ethical responsibility towards their own health and towards controlling costs. While it would be impossible to implement a program that forced people to live healthy lifestyles, it is reasonable to assume that healthier living would lead to lower healthcare costs. Some companies, such as Wal-Mart and the WHO, have stopped hiring employees that smoke to reduce healthcare related costs. Clinical Practice Experience (CPE) RecordGovernment-The Declaration of Independence seems to juxtapose two rights: the right to equality and the right to liberty. Equalitarian emphasize the former; libertarians, the latter. Equalitarian hold that healthcare is a human right; libertarians hold that healthcare is a commodity. Equalitarianism emphasizes the role of government and is more appealing to democrats; libertarianism emphasizes the role of free market and is more appealing to republicans.Physicians-Currently rising premiums and strict requirements are keeping many people from obtaining health insurance. The insurance companies remain profit driven, but the nature of their service should not be profit focused. Adequate healthcare is becoming harder to obtain due to financial hardshipPharmaceutical companies-Pharmaceutical companies also play a key role in the healthcare system because many patients rely on their products. The prices for drugs are rising, and there are no caps to prevent them from reaching extravagant prices. The argument that the pharmaceutical companies need to charge ever higher prices to cover research costs is simply not true. Clinical Practice Experience CPE Record 21/06/2021
2c.𑂽  ●       Review your Advocacy Action Plan Summary in Phase 1, then:o   identify four agenda items for the first Advocacy Action Team meeting.o   describe why these items should be discussed in the first meeting.  Broad healthcare advocacy encourages reforms that put the newest research into practice and cultivate better partnerships within the healthcare industry. It also strives to push for health policies that better account for factors such as poverty, as well as other social and economic circumstances that affect people’s health.When successful, healthcare advocacy that influences public policy improves individual and community health, which reduces health disparities. Such advocacy guides health policy goals, gets priority legislation passed, and secures the funding needed to support the new policies enacted. Clinical Practice Experience CPE RecordFor example, healthcare public policy advocates jumped into action with the onset of COVID-19. They worked to educate policymakers on how to address care coordination, use telehealth, and manage privacy issues to best help patients and save lives. Clinical Practice Experience (CPE) Record

In the past and present, health reform efforts have impacted medical research and education. In response to the AIDS crisis, for instance, advocacy group ACT UP played an instrumental role in getting AIDS research funded. That ultimately led to the development of anti-retroviral treatments available today that keep millions of people alive.
