Welcome to the Clinical Practice Experience (CPE) for this course. The CPE for the Master of Science in Nursing program core courses consists of a variety of semi-structured activities. By completing all the activities and evidence listed in this document, and earning a grade of “Competent,” you will earn 25 indirect CPE hours for this course. Clinical Practice Experience CPE Record
CPE Objective:
In this CPE, you will take on the role of an advanced professional nurse responsible for influencing policy issues facing the nursing profession and healthcare. You will build on knowledge gained in your MSN core course work to develop the skills needed to effectively lead an interprofessional Advocacy Action Team.
In this CPE, you will experience the role of an advanced professional nurse in two phases:
- Phase 1: You will research and plan for advocacy.
- Phase 2: You will develop the structure for your advocacy team.

Written Reflection Summary of your Phase 1 experience As the nurse leader, there are a few techniques available to me to make up for the variance while still complying with set regulations. One method that I will use to balance the variance is to reduce the number of hours per shift for all FTEs during the night shift and day shift. Instead of the 12-hour shift, the FTEs will only do a maximum of 9-hour shifts for the month. Every shift will still have at least 8 RNs in the surgical department. The number of assistant nurses remains as it was before (3), for both night and day shifts. Alternatively, I can increase the use of part-time nurses while reducing the number of FTEs. The idea here is to curb the ballooning wage of FTEs by capping their numbers per shift and the number of hours they work per shift until the variance is balanced. BUILDS TRUST Trust plays an essential role in building effective teams. For teams to work together, they need to know they can trust each other. That they can fall back on each other if the need arises. Moreover, when you build trust among teams, they give each other space and autonomy to accomplish their tasks and make their own decisions. Clinical Practice Experience CPE Record Trust makes people feel safe. When they feel safe, they open up. They let their team members know about their strengths and weaknesses. They are more proactive with their ideas, take risks, listen to each other and then arrive at a consensus. As a result, there’s more collaboration, communication and team members aren’t afraid to expose their vulnerabilities to each other. Clinical Practice Experience CPE Record 2. REGULATES COMMUNICATION When employees work as a team, they communicate. They talk to each other about the task at hand and the best way to achieve the desired result. They strategize, divide themselves into smaller groups, hold discussions and try to finish the task in the most efficient manner. Communication also allows employees to understand their roles and what their peers are doing. When employees know what their team members are doing, they can check up on the progress made and help each other out if someone cannot reach their goal. 3. INCREASES PRODUCTIVITY Teams share the workload. This means that if one member has relatively less work, she can help another team member complete their work. This allows the project to be finished faster, thereby increasing productivity and improving the overall bottom line. Clinical Practice Experience CPE Record The importance of team building is that it improves the individual’s and the organization’s productivity. Because individuals work in teams, they can pick up on new skills and sharpen their existing skills. This improves team performance, makes them efficient and over time, more work gets completed in less time. As a result, organizations can generate more revenue when they achieve their targets and deliver their best.
Health advocacy or health activism encompasses direct service to the individual or family as well as activities that promote health and access to health care in communities and the larger public. Advocates support and promote the rights of the patient in the health care arena, help build capacity to improve community health and enhance health policy initiatives focused on available, safe and quality care. Health advocates are best suited to address the challenge of patient-centered care in our complex healthcare system. The Institute of Medicine (IOM) defines patient-centered care as: Health care that establishes a partnership among practitioners, patients, and their families (when appropriate) to ensure that decisions respect patients’ wants, needs, and preferences and that patients have the education and support they need to make decisions and participate in their own care. Patient-centered care is also one of the overreaching goals of health advocacy, in addition to safer medical systems, and greater patient involvement in healthcare delivery and design. Clinical Practice Experience (CPE) Record Patient representatives, ombudsmen, educators, care managers, patient navigators and health advisers are health advocates who work in direct patient care environments, including hospitals, community health centers, long term care facilities, patient services programs of non-profit organizations or in private, independent practice. They collaborate with other health care providers to mediate conflict and facilitate positive change, and as educators and health information specialists, advocates work to empower others. Clinical Practice Experience CPE Record In the policy arenas health advocates work for positive change in the health care system, improved access to quality care, protection and enhancement of patient’s rights from positions in government agencies, disease-specific voluntary associations, grassroots and national health policy organizations and the media. Clinical Practice Experience CPE Record There may be a distinction between patient advocates, who work specifically with or on behalf of individual patients and families, or in disease-specific voluntary associations, and health advocates, whose work is more focused on communities, policies or the system as a whole. Some companies, such as Wal-Mart and the WHO, have stopped hiring employees that smoke to reduce healthcare related costs. Equalitarian emphasize the former; libertarians, the latter. Equalitarian hold that healthcare is a human right; libertarians hold that healthcare is a commodity. Equalitarianism emphasizes the role of government and is more appealing to democrats; libertarianism emphasizes the role of free market and is more appealing to republicans.Physicians-Currently rising premiums and strict requirements are keeping many people from obtaining health insurance. The insurance companies remain profit driven, but the nature of their service should not be profit focused. Adequate healthcare is becoming harder to obtain due to financial hardshipPharmaceutical companies-Pharmaceutical companies also play a key role in the healthcare system because many patients rely on their products. The prices for drugs are rising, and there are no caps to prevent them from reaching extravagant prices. Such advocacy guides health policy goals, gets priority legislation passed, and secures the funding needed to support the new policies enacted. In response to the AIDS crisis, for instance, advocacy group ACT UP played an instrumental role in getting AIDS research funded.
