Substance-related and addictive disorders are responsible for significant psychological and physical health deterioration in the victims. My client, CK, is a 25-year-old African American male who I have been counseling for the alcohol-related disorder at my practicum site. CK lives with his friends who are also alcoholics in an apartment. He has been taking to alcohol for the past six years since graduating from high school. He frequently takes whisky and tequila shots but can take other types of alcohol with friends. About two years ago, CK became dependent on alcohol such that he could wake up in the morning and take few shots of whisky before starting activities of daily life. CK reports feeling guilt occasionally when sober and thus he wants to quit drinking but when he starts to feel awful and crave alcohol, he ‘loses’ control and any amount of alcohol available would suffice. He reports that 3 three years after high school he used to work on a poultry farm but lost the job due to absenteeism and lateness. He reports no annoyance when his colleagues criticize his drinking.

Therapeutic Approach for CK

CK has been attending counseling sessions for the past six weeks. The counseling sessions are scheduled three times a week.  He attends individual therapy sessions and has shown few signs of improvement for the baseline assessment just after admission into the therapy. His therapy adopted the Prochaska and DiClemente model of behavior change (Liu et al., 2018). The expected outcomes of his therapy included a reduction in the frequency of drinking, reduction in withdrawal and dependence on alcohol, and a positive change in the mindset of the client through reduction of guilt and improvement in time management. Using the Prochaska and DiClemente model CK entered the cycle in the pre-contemplation phase (Hammond & Sharma, 2017 NURS 6670 Week 4 Substance-Related Disorders).

Notably, CK had under the caring mother alone since he was 10 when his father passed away. He has lived with the uncles and aunts since then. This would significantly affect the outcomes of the counseling. A family-based approach would be required to improve his care. CK’s friends are also addicts of alcohol and would require therapy to reduce the maintenance etiologies in his addiction. Group therapy would be appropriate instead of individual therapy.

Conclusion

CK has a maladaptive disorder involving alcohol addiction and dependence. He is in the initial stages of counseling therapy using the Prochaska and DiClemente model. There are maintenance factors in his etiology that would require adjustment in therapeutic interventions. Family-based approach and group therapy would be useful in his therapy.

SUBJECTIVE

Stefanie is a 32-year-old female from Puerto Rico who presents to your office today with complaints of difficulty sleeping. You learn that Stefanie can go for a few days with minimal sleep (about 3 hours/night), but does not seem to be fatigued the next day. Stefanie explains that after 3 days with minimal sleep, she “crashes” and has a good night’s sleep. She states that sleep will be “alright” for a few days, even a few weeks, and then she will have a similar issue with sleep.

You learn throughout the assessment process that Stefanie has had this problem for years. She noticed that it began in college and thought it was just because of the workload and academic demands. However, she found that it persisted after college. She also notices that she has periods where she will engage in increased amounts of goal-directed activity.
