Surgical site infections (SSI) are one of the more common surgery-related complications with an estimated 2 to 5% incidence rate in the U.S. SSIs are associated with increased lengths of stay and higher rates of morbidity and mortality and are the costliest of all healthcare-associated infections (HAIs). A comprehensive approach to quality improvement is necessary to reduce SSIs.

Six Sigma and DMAIC (Define, Measure, Analyze, Improve, and Control) is a robust framework that uses statistical tools for the identification of the root causes of the variations in processes that contribute to SSIs, address and eliminate the variations, and control process improvements and sustainment of the new process for a reduction in SSIs.

With DMAIC healthcare professionals can identify and improve the variables that contribute to the incidence of SSIs and analyze the data to make improvements in the processes and sustain gains. Flow mapping can be used to analyze the existing processes to identify causal factors of SSIs (Shi et al., 2022). A failure mode and effects analysis (FMEA) is another strategy for identifying process failures and recommending improvements (Godley & Jenkins, 2019).

A possible cause of SSIs could be a lack of standardization of perioperative protocols. An Intervention could be to standardize the protocols with evidence-based interventions and processes for certain procedures and utilization of clinical pathways. Another contributor to SSIs could be environmental cleaning and the interventions for improvement can involve education for staff on appropriate cleaning, improved maintenance of airflow systems, and implementation of a protocol for auditing the cleaning processes.

Six Sigma DMAIC is a multimodal process that produces better results than traditional quality improvement methods to achieve performance improvement and reduce SSI rates (Shi et al., 2022). DMAIC is an effective approach for improving established processes. Controllable causal factors such as variations in protocols allow for improvement strategies to be implemented for direct control over the causal factor (Fondahn et al., 2016).
